Dear Ms Roberts,

Re: Mrs Jasmine Thompson, aged 75

Mrs Jasmine Thompson, who has been recuperating from total right shoulder replacement, requires home visits for appropriate care and assistance following the discharge.

Mrs Thompson admitted to hospital with the complains of right shoulder osteoarthritis. Therefore, she underwent the surgery. Post operatively, she has been complaint with physiotherapy exercise regime. Her pain was managed with analgesia and cold compress for four hours in a day. Her wound is clean and dry. Her post-op staples to be removed after 10 days in orthopedic joint replacement outpatient department during follow-up.

Mrs Thompson is a widow and lives alone in single-storey house. Her daughter will stay with her for one month.

Based on the above mentioned circumstances, ongoing care and support for Mrs Thompson would be highly appreciated. She has to wear right arm sling for four weeks along with no lifting. She has to be provided with physiotherapy in outpatient twice and hydrotherapy once in a week. She will require assistance in showering and administration of Clexane injection which needs to be continued for 04 days as DVT prophylaxis. Specialist nurse can be contacted for any concern by calling the hospital.

In case any additional information is required, please do not hesitate to contact me.

Yours sincerely,

Registered Nurse.
