Dear Mrs Penny,

Re: Ms Sheila Cartwright, 69 years old.

Ms Sheila Cartwright who has has unstable Diabetics mellitus along with foot ulcer stablised. She requires further Assessment and treatment after being discharged today.

Ms Cartwright was admitted to hospital on 14 April 2017, with the complaints of unstable Diabetes as well as foot ulcer. She has been treated with oral anti-hyperglycemic agents Glibenclamid 5mg daily and Metformin 800mg t.d.s. For the last 15 years. Small ulcer on her left foot for that Administer the antibiotics therapy (Augmentine Due for the 125mg orally B.D) and daily Saline dressings. The Augmentine Due forte will need to be completion of the course. She require ongoing management and monitoring of the wound for complications.

Ms Cartwright lives alone and has no relatives and close friends to support her. She retired she need from your help in monitoring her medication, blood sugar. Alcohol intake as well as diet and perform moderate exercise It might also help to introduce to her social activities.
