Dear Mrs Kullan,
Re: Mrs Jake Peterson, DOB: 17 March 2001,
Mr Peterson is a student at are boarding school who is suffering from hair loss and associated depression. He requires further monitoring and special exam arrangements from your facility following his transition to university.
Mr Peterson’s parents are working and living abroad. He has been a champion in Karate black belt and swimming.
Mr Peterson has been having intermittent hair loss for the past five years. Initially he oresented with hair loss in patches along with anxiety followed by bullying incident at school for which he was referred to Tichologist, who could not find any artiology and started on steroid cream which he discontinued later on.
He was doing well until December 2018. In December he visited clinic with same complaints and lost 4 kg of weight over the last 2 months. He was reluctant to start natural remedies; therefore homoeopathic treatment was started, which was not effective. Apart from that increase in low mood and distress was noted.
As a result, he could not maintain good relationship with his friends and that affected his sports participation. Subsequently he was referred to school counsellor but failed to attend that far too. In addition he has increased alcohol consumption.
In view of the above, you are requested to take over Mr Peterson. Currently he is anxious about his transfer to university and living independently. It would be appreciated if you could provide support and make necessary arrangements during exam to avoid his anxiety.
Please do not hesitate to contact me for further queries.
Yours sincerely,
Registered Nurse
