Re: Mr Lionel Ramamurthy, aged 63
Mr Ramamurthy, who was admitted on 04 February 2019 has been diagnosed with pneumonia. He is ready for discharge and back into your care tomorrow.
Upon admission, symptoms such as acute shortness of breath with wheezing sounds, persistent cough along with fever , rigors, sleeplessness and generalized aches were noticed.Difficulties in mobilization as well as with activities of daily living was noticed;therefore, frames and assistance were required during hospitalization. Mobilization was also limited to short distances due to increased shortness of breath on exertion.
Presently, the condition was stabilized with oral fluids, good nutrition and chest physiotherapy which need to be encouraged following discharge. Results of inflammatory markers were normal ; however, dry cough, intermittent pain and slowness in mobilization were noticed.
For maintaining good progress, it is recommended to continue regular exercises along with good nutrition. For ensuring postural drainage, it is advised to be in sitting position whenever possible rather than lying down. Paracetamol to be given if necessary for pain management and maintaining warmness is important.
