Dear Charge Nurse,


Re: Mr Amir Akbari, aged 41


Mr Amir Akbari, who has been diagnosed with Guillain-Barre Syndrome, requires rehabilitative care and assistance. He will be transferred into your facility following the discharge today.


On 06 March 2011, Mr Akbari admitted with rapid deterioration of his condition. He had to be in ICU with ventilator support for three days. At the peak of GBS, he could not move his limbs independently. His muscle tone and strength have been decreased; daily physiotherapy program including self exercises has been initiated and need to be continued. Although, he walks with frame, he needs light assistance with ADLs. He had regular plasmapharesis and had five iv gammaglobulin.


Mr Akbari lives in a rented house with his wife and two daughters. He is depressed about his prospects after illness as both are PhD students and they are socially isolated. His wife is not able to cope with financial, study and childcare pressure. He is prone to have depression and anxiety because he has PTSD due to war trauma in mid 1980's; thereby, psychiatric assessment has been recommended.


Based on the above-mentioned circumstances, ongoing care and support would be highly appreciated for Mr Akbari. He has been encouraged to perform limb exercises. Respiratory status and neurological signs have been monitored regularly.
