Re: Ms Sheila Cartwright, age 69
I am writing regarding Ms Sheila Cartwright, who has been diagnosed with unstable diabetes mellitus with small infected left foot ulcer requires follow-up care and appropriate life-style care plan. she is being discharged today .
Ms Cartwright was admitted to hospital on 14 April 2017 with the complaints of non-insulin dependent diabetes mellitus for which Glime 5 mg daily and Diabes 800 mg three times a day has been started and also she has a small infected foot ulcer on her left foot for which antibiotic therapy has been commenced (Augmentin duo for do4 125 MG orally twice a day) and for review at completion of current course. Saline dressing has been done daily at the wound site and also monitored wound margin for signs of complication and has been reviewed for healing progress.
Ms Cartwright is a retired single women who lives alone with a small pension income. Since her retirement,her intake of alcohol has been increased and also dietary quality has been reduced.In addition she has been self administration of hypoglycemic medication.
In view of the above, it would be appreciated if you could monitor Ms Cartwright, medication compliance , blood sugar, alcohol intake and diet. kindly encourage her to involve in moderate exercise program and establishment of social activities such as income producing activity. Importantly involvement of community social service worker, would be beneficial for Ms Cartwright for her lifestyle plan.
