Re: Ms Jasmine Thomas, aged 76
Thanks for accepting Mrs Jasmine Thomas who underwent successful right total shoulder replacement surgery, requires continuing care and management from your service. She is being discharged today.
Mrs Thompson was admitted to hospital with the diagnosis of right shoulder osteoarthritis for which she underwent and above-mentioned surgery.Therefore x-ray was taken after the surgery to confirm the position and also post of bloods were within normal limits. Currently she is voltraren 500 mg daily (it was hold 14 days preoperatively) panadine forte 500mg 2 tablets every 6 hourly and LMWH subcutaneous for four days as a DVT prophylaxis. Wound and drain site where clean and dry and cold compression applied on the right shoulder . She has been complying with physiotherapy and received post operative education regarding care after her surgery.
Mrs Thompson is a widow, her daughter will be staying with her for a month post-surgery. She has been instructed toavoid lifting for four weeks while her arm remain in sling. She requires assistance for activities of daily living. Physiotherapy exercise has to continue as per TSR protocol. She has a follow-up appointment with Physiotherapy twice per week and hydrotherapy once per week at outpatient clinic. She also has a follow-up appointment in Orthopaedic joint replacement outpatient department for staples removal.
It would be appreciated if you could assist her for daily living activities and administration of LMWH. Continue cold compression 4 hours per day.
