Dear Ms Harry,


Re: Mr Robert Downey, aged 66


I am writing to refer Ms Amelia Harry, who has been diagnosed with a diabetic foot ulcer, for your continuing care following her discharge today.


Ms Harry was admitted to the hospital with complaints of a right toe ulcer associated with itching and pain. The foot ulcer appeared swollen, reddened, with purulent discharge. Culture swab revealed severe infection with 3 aurei. During Hospitalisation, the foot ulcer is cleaned and dressed regularly. Antibiotic therapy with amoxicillin has been initiated.


Ms Harry has had hypertension and DM since 2010 and 2000 respectively and is on medications Ramipril 10mg, glimepiride 1mg, and Metformin 500mg. Non-compliance with the medication has been reported.


It would be highly appreciated if you could conduct a home visit for dressing his foot thrice a week. Kindly monitor his medication compliance and educate him regarding foot care and foot hygiene.
