Re: Lionel Ramamurthy, aged 63
I am writing to update the current health status of Mr Ramamurthy who is being treated for pneumonia. He requires continuing care and monitoring following his dicharge tomorrow.
Mr Ramamurthy was admitted to the hospital on 04 February with suggestive features of pneumonia. On admission he was very weak and ambulated short distance with pick-up aid. He also had increased shortness of breath on exertion and required assistance to perform activities of daily living.
During hospitalization, Mr Ramamurthy showed good progress. He was afebrite and his inflammatory marker return to normal. Currently, he ambulates independently.
On subsequent days, he has chest and abdominal pain along with dry cough. Therefore, he was provided with chest physiotherapy and advised to sitting position to facilitate postrual drianage. Additionally he was reviewed by the dietician and has gained 1.5kg of weight.
Upon discharge, keep him warm. Please monitor his dietary pattern and ensure good nutrition such as egg, fruit and vegetables. Please encourage him to increase fluid intake. Please note that paracetamol can be administered if he has chest or abdominal pain.
