Dear Sir/Madam,


Re: Alicia Nesbitt, DOB: 22 April 2016


I am writing refer Alicia, a 6-day-old baby who has been diagnosed with low birth weight and opoid dependence. She requires follow-up care from your facility as she is being discharged today.


Alicia’s mother Gemma has been heroin dependent for two years and recently worked as a sex worker. She is not in a good relationship with Alicia’s father due to domestic violence and currently living with her mother who is supportive.


Alicia weight was 2.0kg at birth. On subsequent days poor bonding between mother and child was noted. Most of the time Gemma required prompting to care for her child. As a result Alicia had decreased appetite and bottle feeding was insisted. Alicia now weighs 2.3kg, which needs to be monitored. Apart from that both mother and baby has completed heroin withdrawl without complications.


Therefore, please take over Alicia from this point on. You are requested to monitor Gemma coping and psychological state. As she is going to live with her mother, ensure safe environment fro Alicia and if risks present kindky report to social worker. Your assistance also required to arrange daily visits until Alicia’s weight and feeding is stable. Please arrange liase with drug and alcohol team for Gemma to decrease the risk of heroin.
