Dear Mrs Penny,
Re: Ms Sheela Cartwright, aged 69
Ms Sheela Cartwright Who has been diagnosed with unstable diabetes mellitus and small ungectial legal foot ulcers being regesred to you. She requires ongoing care and support following her discharge today.
On 14 April 2017, Ms Cartwright was admitted to the hospital with aforementioned condition. He was managed walk daily Saline dresing and antibiotic therapy (Augmentin Duo Forte), which needs to be review for the completion of current course. In addition, glimenclamide, 5mg once in a day and Metformin 800mg, three times daily have been administered. Problems with self administration of hypoglycaemic medication was reported; therefore monitoring of medication complaints is required.
Ms Cartwright, who lives alone in own house and retire on small pension, which is less than the pre-retirement income. Increased alcohol intake and decreased dietary quality were reported since her reitrement, also she has no relatives and friends. She has had non-insulin dependent diabetes mellitus for 9 years.
In regards of discharge plan, regular monitoring of blood sugar levels, alcohol consumption and diet regimen would be profoundly beneficial. Involvement of a commonly social worker is recommended in order to suggest establishment of income-producing activities. Kindly encourage her to involve in social activities and moderate exercise programmes to continue her general health and well-being.
