Re: Ms Sheila Cartwright, aged 69
I am writing to refer Ms Cartwright who was diagnosed with unstabled diabetes mellitus and infected left foot ulcer. She requires ongoing care and support from your service. She is set to be discharged today.
Mrs Cartwright admitted on 14 April 2017 and diagonises above condition. During hospitalization, she was treated with Glibenclomide, metaformin, Amoxycilline. Wound dressing done with saline and monitor for complications. Noted good healing progress.
Socially she lives alone in own three bedroom home. She receives small pension. Se has had non insulin dependent diabetes mellitus for the last nine years. Her dietary quality had been reduced and has problems with self administration of hypoglycaemic medications. She is dont have relatives or close friends additionally she is inactive in other activities.
Kindly monitor her with medication complaince as well as diet, blood sugar level and alcohol intake. Please encourage her to perform moderate exercises and ivolce her in social activities. Please make arrangement to make an appointment with community social workers for the establishment of income producing activity.
