RE: Mr Lionsl Ramamurthy, 63 years old
Mr Ramamurthy was admitted in Newton Public hospital on 4 february 2019 being diagnosed with pneumonia. He is now ready to be discharged back into home your care retirement
On admission Mr. Ramamurthy presented with acute shortness of breath with wheezing sounds, persistent cough going along with chest and abdominal pain, fevers, rigors, sleeplessnes and generalised ache and seemed weak using mobilising with pick up frams,
buring his stay in hospital, was encourage on oral fluids to lower fever and to adhere on proper nutrition as he gained weight has by 1.5kg after being reviewed by disticion beep breathing and coughing exercises were encouraged to improve breathing pattern.
Please encourage Mr Ramamurthy to continue with breathing excersices and to ambulate as per physiotherapist review, Advice him to Keep on lying down to ensure postural drainage. He is being discharged with paracetamol for chest and abdominal pain and use it if necessary.Keep on advicing him on proper nutrition with lots of to fluids and vegetables, Please ensure that he keep warm all the time.
