Dear Nurse Manager


Re: Miss Benyl Casey, aged 72


Mrs Casey, who has been recuperating from a left hemiarthroplasty requires ongoing care at your facility after being discharged today.


On 4 February 2014, Mrs Casey was admitted for which fell down at home. Therefore, she underwent hip replacement. Post-operatively, three units of packed cells were transfused. One episode of hypertension for which ramipril 5 mg dose was decreased. Intravenous antibiotic course was completed. Alternative staples has been removed and dressing changed. anti-embolic stockings has been recommended for 14 days. Mobilizing short distances with pick-up-frame and one person assistance is required. Abduction pillow whilst resting and activities of daily living requires full assistance.


Mrs Casey is widowed, who lives alone in 2-bedroom house with stairs to entrance . She has 2 children, son lives locally and daughter is in interstate. Her son visits twice a week was mentioned.


As per the above circumstances, discharge medication ramipril 5mg daily and paracetamol 1gm is needed were advised. Continue monitoring blood pressure twice a day for atleast 3 to 7 days. Consequently, anti-hypertensive medication dosage to be adjust. It is vital to know check haemoglobin level and on the 10th day remaining staples to be removed. Therefore, wound can be exposed. Encourage for rehab therapy after your assessment. Family has been involved about transfer and hospital transport is arranged.
