Re: Mrs Mavis Brampton, age 72 years


Mrs Brampton who has been diagnosed with Pleurisis. Mrs Brampton requesting monitoring and ongoing care be arranged for community nurse for continuing her cessation of smoking.


Mr Brampton 10 January 2011 since overweight and 29 had exray and SV Amoxycillin sith supplementation, oxygen therapy. Vital signs slightly raised. Due hospitalisation low diary diet and adviced about nicotine patches. Moreover productive course sputum culture done. Pravastatin and celecoxlb 100mg/day. Deep breathing exercise started; However using nicotine patches and Zyban patches 150mg two per a day.


Mrs Brampton widowed since 25 years. Mrs Brampton attends the local community centre 3 times a week to play bingo. She is smoker since 18 years of age. She is smoking 10 cigarettes a day.


Kindly monitor her quit smoking plans, explaine for zyban side effects for (eg) dry mouth, difficulty sleeping, if any side effects occur should be stopped. Zyban to be withdrawn after 2 months she can continue until smoking addiction is under control.
