Dear Ms Davone
I would be greatful if you could provide advices regarding normal resume of daily activities, during a home visit for Mr. Peter Hendrick, who is being discharged today after an emergency angioplasty, following an admission to the hospital for myocardial infarction.
Mr. Hendrick is married and has three children. He is a truck driver with mixed shifts. He prefers junk foods and has body mass index of 29.
On O2 July 2019, Mr Hendrick was brought by an ambulance with continuous chest discomfort and heaviness in both arms. Blood reports showed high cholesterol level, and ST segment elevation was revealed in electrocardiogram. Therefore, an urgent operation was taken place with direct stenting to proximal left anterial descending artery. Post operatively, he was educated regarding diagnosis and modification of risk factors such as cholesterol and weight loss. Physiotherapist and dietician reviews were done.
It would be appreciated if you could do a home visit for Mr Hendrick, and educate him gradual resume of normal daily living activities and work. Please note that he is restricted for driving for 6 weeks.
