Dear Physiotherapist,
Re: Mr Stephen Kerry, 16 June 1963
I am writing to request home visit from your facility for Mr Kerry, who had serious injury to knee to perform mobility.
On 12 July 2011, Mr Kerry fell down in the stairs and injured his right knee. As a result G.P requested for pain management,daily dressing and assistance for his daily activity.
During his first visit on 13 July 2011, Mr Kerry was bed bound and refuse to take shower. He complained of pain in right knee which managed with paracetamol. Performed dressing and additionally warm compress applied.
Mr Kerry visited again after third day, noticed the redness and free from infection. He is able to walk little distance with the help of wife. Later on his knee was well healed and he started to walk with help of walking sticks.
In view of the above I would greatly appreciate it, if you could discuss with his wife (Mrs Jennifer), regarding the need for a physiotherapist to Mr Kerry.
