Re: Mr Gerald Baker, a 79-year-old patient
I am writing to update you regarding the current health status of Mr Baker who is recovering after a left total hip replacement. He requires continuing care and monitoring as he is being transferred back to your facility.
Post-operatively Ms Baker’s hemoglobin dropped to 70gm/dL; therefore three units of packed red blood cells were transfused. His current hemoglobin is 112gm/dL; however, he requires monitoring for anaemia. Consequently daily dressing on surgical site was performed.
Following curfew Mr Baker was recommemded on Aspirin 100mg daily. In addition to his usual treatment for hypertension. His pain was managed with panadeine forte. On subsequent day, he was referred to occupational therapy and was able to perform range of motion exercises along with stretching anf strengthening. He is also able to mobilize around the ward with the help of wheelie-walker without difficulty upon discharge Mr Baker requires daily dressing on surgical site to prevent his skin integrity. Please monitor his medication complaince and exercise regime. He requires wheelie-walker, wedge pillow and toilet raiser. The wheelie walker and wedge pillow are provided from the hospital and the toilet raiser has hired for two weeks by the social worker with the help of medical supplier. Please note his staples to be removed on 21st September and he has to visit City Hospitak Clinic for follow-up FBE and UEC tests.
