I am writing to refer, Mr Peter Hendrick who under emergency angioplasty following myocardial infarction. He requires occupational therapy Services at his home. He us being discharge today.
Mr. Hendrick was brought to the hopsital by ambulance with the complaints if heaviness in both arms, discomfort in chest and jaw. Series of investigations were performed including LCG which revealed ST segment elevation. And his ejection fraction who 35%. So he underwent emergency surgical procedure. His blood reports revealed high cholesteol level. Educated him regarding the importance of reducing weight as well as to control the cholestrol.
He works as a trcuk driver and has shifted. He leads a Sedentary life style. He has had mild arthritis and asthma but no exacertations for the last 5 years. He has been by the dietician inorder to reduce the cholestrol and weight. Cardiac exercise initiated by Physiotherapist. It would be greately appreciated, if you could assist Ms. Hendrick. Kindly review his driving and normal activities. Please note that, driveing is not recommended for 6 weeks. Kindly monitor whether he is able to go back to his work.
