RE: MR GERALD BAKER AGED 79


I am writing with regard the discharge of Mr Baker who was admitted in city hospital on 3 September 2010 due to surgery of left a total hip replacement following osteoarthritis since 2008. He requires your ongoing care back to nursing home.


Post operatively, Mr Baker was disorientated to time and place which might be related to anaesthetic as there is no signs of dementia marked, therefore ongoing observation recommended. Haemoglobin dropped after operation to 72 though stabilized to 112 following transfusion with 3 units of packed red blood cell. After operation, mobility seemed good as he can walk length of word without difficulty using a Wheelie-Walker.


Ongoing monitoring of anaemia and management of hypertension was recommended for Mr Baker. He recommenced on Aspirin 100mg Mane post operatively and to continue with Ramipril 5mg mane and class monitoring of Panadeine Forte 2 gid PRN. Daily dressing of incision site may be managed. Occupational therapy and range of motion, stretching and strengthening exercises was part of management.


Please remind Mr Baker to remove Staples in two weeks on 21 September 2019. Encourage him to continue with exercises programme to present skin integrity. May you follow up FBE and UBC test at city hospital.
