Re: Mr Gerald Baker, aged 79
I am writing in update you the current health status of Mr Gerald Baker who has been recovering from left total hip replacement surgery. He requires ongoing care and management following his discharge today.
Post-operatively, Mr Baker was managed with daily dressings, occupational therapy along with strengthening and stretchening excuses consequently is mobility has been improved and started to walk with the use of a Wheelie Walker without any difficulties. While the recovery period following surgery, he developed disorientation, which was probably due to the effect of anaesthesia,therefore continued observation recommended. In addition, three units of packed red blood cells were administed to manage his post surgery anemic with haemoglobin level of 72, current level is 112, however ongoing follow-up is required.
Post-surgery tab asprin 100mg at morning was recommended along with tab ramipril;5mg to manage his ongoing hypertension. He was also administered panadene forte; two tablets for every six hours, which needs ti be monitored.
Based on the aforementioned circumstances, the continued care for Ms Baker including excise programmes and skin integrity preservation care would be highly beneficial. Ofnote? The staples needs to be removed on 21st if this month, also he has to attend this hospital for a follow-up visit with FBE and UEC test results.
Additionally , a wheelie walker and pillow will be provided from the hospital and a toilet raiser will be organised by the hospital social worker for a pension two weeks from a local medical supplier.
