Re: Mr Peter Hendrick, DOB: 21 May 1978
I am writing to refer Mr Peter Henrik, who has been recovering from myocardial infarction. He requires home visit to provide advice resume less work and activities of daily living.
Mr Hendrick was admitted in hospital on 2 July 2019 with the complaints of constant chest discomfort, for a heaviness associated with intermittent for discomfort. Consequently, the diagnosis was comfirmed and he underwent on emergency angioplasty. During hospitalization, he was reviewed by physiotherapist in order to provide cardiac exercise programs as well as a dietician regarding weight reduction he attained good progress. However he is infused about the diagnosis. Therefore, he was educated regarding the his condition and the importance of rest modifications.
Mr Hendrick is married and hs three children. He workds as truck driver and doing mixed shifts duties. He has had dyslepidaeme which id nkt treated and mild osteoarthritis.
He also had a strong family history of cardiac disease.
Based on the aforementioned circumstances your support and guidance of Mr Hendrick in return to his work would be highly appreciated. Ofnote, he needs to br refrained from driving for the next 6 weeks.
