Mr. Hendrick, D.O.B- 21/05/1972


I am writing to refer Mr Hendrick who had been diagnosed myocardial infraction, admitted in the cardiology unit at St. Patrick’s hospital Melbourne Victoria 3006, need home visit for provide advice on returning to work with daily activities.


Mr Hendrick admitted the hospital due to complained of heaviness in both forearms and feeled discomfort in chest. His dietary history that eats fast food with non drinker. Five years ago he had mild osteoarthritis and mild asthma. He advised the following medicine Atorvastatin 40mg, Metoprold 23.75 mg, Ciliazipril 0.5mg, Aspirin 100mg, Ticegvalor 90mg Glytrin spray pon

Incase of chest pain occur.


Mr Hendrick need a dietician for his weight loss to provide diet list. He also reduce the cholesterol levels by taking healthy food. He required a physiotherapist for his exercise program. He has to advise to change his lifestyle for prevent further heart attack. As he is a profession of driver, needs 6 week rest restriction of drive. Please provide advice on returning to work as well as normal activities with the help of successful rehabilitation.
