Re: Ms Tabitha Taborlin, aged 45
I am writing to refer Ms Tabitha Taborlin, Who has been diagnosed with type 1 diabetes mellitus,. She requires continuing care and further management. She is being discharged today to your facility.
Mr Taborlin was admitted with complaints of diabetic ketoacidosis, her glucose 530 mmoL increased: for which recommended intravenous fluids and insulin administrated.
Ms Taborlin has had long history of type 1 diabetes since seven years and non-compliance with insulin regimen and prescribed 45 units lantus night and prepardial correctional Scale Humalog with 12 unit nutritional baseline.
Ms Taborlin has a school teacher lives alone. She is not following any exercises, her BM 18.2. Smoking moderately(2a/d).
Kindly request diabetic education monitoring glucose level and insulin administration. Smoking cessalean education, additionally, microvascular, microvascular complication of diabetes and also stricter glycemic control and possible insulin pump follow up for one minute.
