Re: Mr Sean William, aged 72 years

Dear Ms Beckinsale

Mr Sean William, who has persistent breathing difficulty, is being referred for appropriate instructions of inhaler usage.

Mr. William, widower, lives alone and has two children. He has chronic obstructive pulmonary disease, for which he takes salbutamol and ipratropium.

On 06 February 2019, during home visit, Mr William appeared with exertional shortness of breath. General Practioner discontinued the medications due to it's ineffectiveness, and was commenced ventulin HFA inhaler 2 puffs, under supervision.

Today, Mr William still experienced the same symptoms. Discussion regarding proper use of inhaler was done, but he claims that the dose and procedure was taught differently for him.

It would be appreciated if you could provide appropriate information and essential reinforcement for the correct use of inhaler.

If you have any further queries, please do not hesitate to contact me.

Yours sincerely

District Nusse
