Dear Ms Dobson,
Mrs Silvia Rule, who has been diagnosed with Bacterial chest infection, requires ongoing care and monitoring following the discharge today.
On 10 June 2021, Mrs Rule was presented with the suggestive symptoms of pneumonia such as fever, cough, wheezing and shortness of breath. A series of investigations including CT scan were done, which confirmed the diagnosis and pneumonia has been ruled out. Oxygen through face mask and intravenous Amoxicillin,500mg have been commenced.
During admission, Mrs Rule was reluctant, aggressive, shouting and restless. She is diagnosed with Alzheimer's disease. Today, her symptoms were subsided and she is fit for discharge.
It would be appreciated, if you could monitor Mrs Rule's Medication complaince, fever, fatigue and breathing problem. Tab Amoxicillin, 500mg,thrice daily should be continued for a week.
