Re: Mr Philip Satchell, aged 73
I am referring Mr Satchell who is being transferred to your area for the better treatment of multiple sclerosis. He requires continuing care and support from your facility.
Mr Satchell has been visiting our health centre with multiple health issues. He has had type 2 diabetes and poor complaince with diabetic medication as well as diet. Although he is recovering from multiple sclerosis; however he requires continuous monitoring for frequent relapses.
During his recent visit he was noticed with bilateral leg ulcer which is positive for Staphylococcus infection. Therefore, he was commenced in IV antibiotics alongs with metformin for diabetes and daily dressing for hith leg. Additionally he was educated regarding the importance of diabetic regime and medications.
Socially he is a widower and lives alone in a public housing and receives aged pension. Eventhough his sons visit him occasionally, they maintain a good relationship with their father.
Upon transfer you are requested to switch to oral antibiotics, but continue same diabetic medications and dressing twice daily. A referral to the Prince of Wales Diabetic Clinic is necessary for medication review and to reinforce education. Kindly facilitate a referral to the neurologist for multiple sclerosis follow-up. Finally it would be appreciated if you could connect him with community services to enhance coping his social network.
