Re: Mr Hendrick, DOB: 21 May 1972
I am writing to refer Mr Hendrick, who was diagnosed with myocardial infraction. He requires continuing home care until return to intervention he is being discharged today.
Mr Hendrik was admitted on 02 July 2019 with complaints of BIBA for two hours intermittent discomfort jaw and chest, heaviness in both forearms. Therefore imediately intravenous morphine 10mg. Aspirin 300mg chewed, Glytrin Spray into 3 times.
Mr Hendrik undergone investigation Electro Cardiogram. Its shows ST elevatiob. He underwent emergency angioplasty direct Stebtcing to provide LAD done. Ejection fractions 35%. Additionally blood reports shows high cholestral level. Mr Hendrick confused re diagnosed with near death exposure risk factors high cholestrol and weight loss.
To view of above circumstances, additionally supportive therapy include physiotherapy for cardiac exercise programme and dietician for weight loss and reduced high cholestrol level for which recommened medication atorvastein 40mg on metprot 23.75mg OD. Cilozepril 0.5mg on and oral Ticogralor 90mg two lime Glytrin spray as require. Kindly educate no driving for 6 weeks and review with normal daily activities.
