Dear Mr Hyman


I am writing to refer Mr Hyman who has an impaired balanced gait due to a Cerebrovascular for the past two years. He requires your physical therapy for hus speedy recovery.


On 12 July 2011, he presented with a complaint of worsened injury in the right knee following a fall while climbing downstairs, for which he has been visiting The Home care for wound dressings and showering daily. His wiund healed without any infection.


Subsequently, follow-up visits were reported he was being able to walk a short distance with help of his wife and progressing well. Unfortunately, he was complaining of pain while walking for which paracetmol was given. After four days, in his review he has been using walking aid for support, His wife concerned and requested physiotherapy sessions in their home for a speedy recovery.


It could be greatly appreciated if you could assist him with exercises. It could be beneficial for hum to involve the outdoor physical activities.
