Re: Mr Phillip Satchell, Aged 73


Dear Nurse,


I am transferring Mrs Philip Satchell into your care as he is moving to Maroubra for treating Multiple Sclerosis by a neurologist.


Ms Satchell lives alone and receives a small pension. He is currently stable. However, he requires continuous monitoring for signs of Multiple Sclerosis relapse. It might be helpful if you could refer to neurologist by your GP for his follow up care.


In terms of medical history, he has been diagnosed with type 2 diabetes for which he takes metformin twice a day. While in our care, he was educated about diabetic management. It is noted that, he us not complying with a dietary pttern and with medications. In addition, he has an ulcer in both limbs and is also infected with Staphylococcus Aureus including pus formations frequently. Therefore he has been commenced with IV antibiotics followed by oral antibiotics to continue along with antidiabetic agents. Kindly arrange a referral to the Prince Wales Diabetic Clinic to review medications and reiterate diabetes education.


It would be greatly appreciated if you could arrange in community services to enhance him coping and social network.
