Re: Ms Margaret Helen Martin, aged 81
Ms Martin was admitted in Greenville Publuc Hospital on 15 July 2017 for second operation of angioplasty therefore, requires your professional assisstance on her anticipated needs. She is being discharged back to home.
Ms Martin’s First angioplasty was being performed in 2008 following her diagnosis of angina and coronary artery disease which has been managing it with aspirin and clopidogrel. During her discharge cardiologist’s report stull pending. She has a family history of coronary artery disease. Due to having meals on wheels, she is overweight with the BMI of 29.5. She is taking metropolol (Betaloc) and Ramipril (tritace) for management of hypertension where as for hypercholestenomia of 8.3, she is using atovastatin (Lipitor).
Post operatively, seemed to be recovering well and tolerating light diet and fluid though there us swelling at catheter insertion site but no signs of infection observed nor bleeding noted. She seems anxious on her return to home focusing on her health problems and having intermittent contact with family members.
May you kindly arrange home visit by a community health nurse to monitor adherence to post operatively medication, exercise and diet regime. Arrange regular monitoring by doctor dietician and physiotherapy. Arrange regular family visits to monitor progress.
