Re: Mrs Victoria Flangan, aged 88

I am writing to introduce Mrs Victoria Flangan , who has been suffering from dementia, requires continued care and management at your facility.

On 16 February 2008, Mrs Flangan was admitted to hospital with the complaints of confusion, aggression and paranoid behaviour, which was due to the side effects of medication (levodopa with carbidopa), that she takes to manage Parkinson disease. Consequently, she is commenced on bromocryptine mesylate; 1.2 mg at the night time for the initial week and the current dosage is 1.9 mg two times in a day.

Additionally, physiotherapy was provided to improve her physical mobility as well as speaking and swallowing abilities, which needs to be continued.Eventhough, her condition is improved with the change of medication, she is disoriented and has memory loss. She also wears glasses and hearing aids ; and mobilize with the use of walking aids.

Mrs Flangan lives independently with her 89-year-old husband, Mr Tom , who has difficulties to manage her at home due to her aggressive behaviour and disorientation.

Based on the aforementioned circumstances, follow-up care and assistance for Mrs Flangan would be profoundly beneficial .kindly provide a diet which is easily chewable and digestable. Additionally, toilet modifications are recommended to avoid any falls.
