Re: Nasser Ali, DOB: 4 February 1964


Mr Nasser Ali, who has been diagnosed with myocardial infraction due to He was underwent coronary Artery bypass graft. He requires continuing care and assessment. He his being discharged today to your care.


On 09 February 2012, Mr Ali was admitted to the hospital with complaints of acute substrenal chestpain radiating to ledt arm, pain in Jawa and digptoresis, dizziness and shortness of brocolli, he has been diagnosed with myocardial infraction, for which, he was treated with seroptolumin anticoagulants and anti-cholinorgic medication, additionally continuing ECG monitoring. He underwent angioplasty on 10 February 2012.


On discharge Mr Ali was educated about medication as regularly, arrange for dietician to provide dietry advice. He has been reconnect quiet smoking programme and physiotherapy for particularly deep breathing exercises.


In view of the above circumstances, it could be more beneficial if you could arrange community social worker to provide support service to the family to ensure a smooth transition to normal life.
