Re: Mr Peter Dunbar, DOB: 18 March 1932


I am writing to introduce Mr Peter Dunbar, who has been suffering from type 2 diabetic mellitus, requires regualr monitoring and suppirt following his transfer to centreville to live with his daughter.


Mr Dunbar has had diabeties since 2016, which was initially managed with diet and exercise regimens. However, non-complaince with diet was reported, following the death of his wife. Eventhough, he was prescribed with metformin, intermittent medication usage were reported and he complained of poor memory. Consequently, he was educated regarding the importance of regular usage of medications, diet modifications and strategies of memory aids.


In June 2018, he was hospitalised due to myocardial infraction and was commenced on sotalol along with warfarin for the management of atrial fibrillel which developed following the heart attack.


During susbsequent visit signs of diabetic neuropathy were noted an has restricted mobilization. Currently mobilization is possible with the aid of walking stick. Also,poor dietary management and exercise tolerance were again noted. Therefore, re-education on modification adherence were provided and suggested the alternative methods such as meals delivery servuces as consider to live with his daughter.


Based on the aforementioned circumstances regular diabetic monitoring and continuous encouragement of medication complaince for Mr Dunbar would be profoundly beneficial. Additionally, edcuation of takights regarding his dietry needs and management in recommended.
