Re:Mrs Victoria Flangan, aged 88
I am referring Mrs Victoria Flangan, who was diagnosed with dementia, requires ongoing care and appropriate management following her discharge today.
On 16 February 2008, Mrs Flangnan was admitted to hospital with the complaints of confusion, aggression and paranoid behaviour. Assessment and observation have been initiated every four hourly. She has been commenced on Bromocryptpine mesylate,1.9 mg, twice daily. Physiotherapy to improve physical movements, muscle tone and exercises to strengthen the muscles involved in speaking and swallowing have been provided and need to be continued.
Mrs Flangnan lives with his husband who is old and he finds increasingly difficult to cope with her aggressive behaviour. Disorientation and walking at night to dress for work were noticed; therefore, she will be transferred to nursing home. She uses glasses, hearing aids and stick or frame to walk.
Based on the above-mentioned circumstances, it would be appreciated if you could arrange elevated toiled seat, upright chair with arms and back elevated for Mrs Flangnan. Rug on floor to be avoided and she may slip-on shoes. Foods that are appetising, easily chewed and swallowed one are recommended.
