Re: Mr Peter Dunbar, Date of birth: 18 March 1933
Mr Peter Dunbar, who has been diagnosed with type-2 diabetes mellitus, requires ongoing monitoring of his diabetes and encouragement regarding complaince with medication and dietary pattern. Mr Dunbar is moving to centreville to live with his daughter.
Mr Dunbar has had diabetes mellitus for the last three years. He has managed with diabetes diet and physical exercises. Depression and grief have been experienced after his wife’s death. In additon, non-complaince with medication and diet has noticed. Consequently, oral hypoglycaemic drug has prescribed. Education was provided regarding the importance of taking medication and potential adverse effects of intermittent dosage of medication.
Last year, signs of diabetic nueropathy were observed for Mr Dunbar. Ambulation is possible with the support of a walking stick. Exercise tolerance and restricted mobility were noticed. Alternative arrangements such as community- based meal delivery service, moving with adult children and retirement village were suggested.
Mr Dunbar has a history of myocardial infraction. His current medication are metformin 500mg three times a day, ramipril 5mg daily, warfarin 3.5mg and sotalol 40mg daily.
Based on the aforementioned circumstances, regular monitoring and encouragement would be beneficial for Mr Dunbar. Dietary changes and medication have been advised to his daughter. Food will be provuded by his daughter once he moves to centreville. Reduction of salt, sugar , alcohol and fatty food has recommended but he refused.
