Re: Mr Gerald Baker, aged 79


Mr Gerald Baker, who underwent a left total hip replacement, requires continuing care and appropriate management following the discharge back into your care today.


Post operatively, Mr Baker was recommenced on Aspirin 100mg daily, Panadeine Forte 8 tablets per day and also ongoing with hypertensive medication. Daily dressings were applied to the surgery site. In addition, he undergo series of range of motion, stretching and strengthening exercises as well as occupational therapy provided for a fully recovery. He is able to walk with the help of wheelie-walker without having any difficulties. Haemoglobin level has changed from 72g/dL to 112g/dL after transfusing 3 units of packed red blood cells.


Currently, Mr Baker’s medication requires to be monitored. Participation in regular exercise program needs to be encouraged. Wheelie-walker and a wedge pillow provided by the hospital and a toilet raiser has been hired from social worker for 2 weeks.


Please note, Mr Baker has a follow-up of blood test and staples removal after 14 days.
