Re: Maria Joseph, aged 39
I am writing referred Mrs Maria Jospeh who was suspected enhacranial pathology space occupay lesions due to history of fall. She requires urgent assessment and appropriate treatment. She is transder to your care.
On 15 May, 2011 Mrs Joseph was admitted with complaint of fall down at home with severe headache left side, injured her right arm, bruises on left leg, slurected speech and half unconscious. On observation her Bloos pressure 150/90 extension 4/5 power left leg knee flescion 4/5.
Mrs Joseph first presented to the hospital with frontal headache due to nausea and blurred vision. On assessment probably due to excess tension and personal dilemma. She was recommended paracetamol 500mg for four hour.
Subsequently, Mrs Joseph continuing symptoms her condition not improved, additionally she has slight paraesthesia for which Pethedeine 100mg intramuscualr infraction and maxoion 10mg recommended. Although she was suspected migraine.
In view of the above circumatances. Therefore your urgent assessment and intervention would be highly appreciated. If you have any queries please do not hesitate to contact me.
