Dear Mrs James,

Re:- Mr Mathew Philip, DOB-29/06/1962

I am writing this letter to refer Mr. Philip into your facility to request a therapuetic counselling for him and his wife as they adjust to retirement and he needs link with services in the community.

Mr. Philip has been diagnosed with severe depression and anxiety disorder since 2006.He had received treatment of electroconvulsive therapy and antidepressant which was unsuccessful.Besides, he has been suicidal only for a short time in the last three years and he did not meet public mental health eligibilty criteria.Hence, he attended private pychiatrist two years ago.He takes norvosac 10mg everyday.

In addition, he presented to the clinic because his blood pressure was extremely unstable last week.Also, he has a potential for suicidal thoughts.Socially, he can afford a high quality treatment as he has been in demanding administrative position.He was a hard drinker during his 20s and 30s.He has a 15 years old son, is suffering from down’s syndrome and he permanently lives in a disability centre.His daughter is 23 years old and she overseases on military service.

In view of the above, He and his wife are on the advice of his pychiatrist as they are considering early retirements and also applied for a disability support pension.It would be greatly appreciated if you could provide necessary service to the patient.
