Re: Jake Peterson, 18 years old

This letter is to introduce Mr Jake Peterson who requires monitoring and special seating arrangement during examination on transition to the university owing to his alopecia and associated depression.

Mr Peterson is a boarding school student and is about to start study at university. Since March 2014, he has been experiencing intermittent alopecia and disturbed metal health status, which includes anxiety, personality and mood changes. He had tried different streams of specialties to treat the condition; however, he had no compliance with medication. He was observed symptom free for about four years during the period from 2014-2018.

Mr Peterson presented again in December 2018 with worsened symptoms. Weight loss of 6 kg in two months was also noted. In addition, he was appeared isolated. He was referred for counseling which he did not attend. His alcohol consumption is increased to minimum 28 units per week. He required special seating arrangement during his recent examination

Mr Peterson’s parents live abroad and he has family history of depression. Though, he is a sport person but presently not participating in sports.

In view of the above, it would be appreciated if you could provide special attention and support Mr Peterson by arranging exceptional seating area during his examination. Close observation for his condition is imperative.
