Re:Ms Patricia Styles
Dob: 27 April 1957
Ms Patricia Styles requires urgent evaluation and treatment for pericarditis at your facility. She is being transferred today.
On 23 August 2019, Ms styles was admitted to the Green Valley Hospital following fever, chest pain ,generalized weakness and malaise. On examination, her vitals were elevated. She underwent multiple test such as Full blood examination which was abnormal, Throat swabs and Electrocardiogram that revealed viral influenza type B and the pericarditis . During hospitalization, she was commesed with IV saline and Ibuprofen 600 mg every 8 hourly. Ms style was advised for self - care at home for which a nurse was arranged on 30 August 2019.
Today,during home visit, Ms style was unwell following chest pain and shortness of breath. Vitals were elevated with low grade fever of 38 degree celcius, Heart rate 115b/m .
Ms Style has had hypertension and diabetes since 2011 and 2013 respectively for which she has been taking Queen April 40mg twice a day orally;tablet metformin 500 mg twice daily and tablet gliciazide 30mg once a day.
Based on the aforementioned circumstances,urgent assessment and management for Ms Styles would be profoundly beneficial.
