Re: Ms Mary Bell, aged 66
I am writing to update you the current health status and accomplish her medical and social needs. She was diagnosed with unstable diabetes mellitus and small infected foot ulcer being discharged today. Ms bell is retired
from managerial position and lives alone. He is getting small amount pension, however no friends and relatives. Therefore, he consume more alcohol and changes in dietary habits. In addition there will be problem with
self-addministration of hypoglycemia medication. the condition was treated with glibenciamide 5 mg daily, metformin 850 mg thrice daily, amoxycillin 875/125 mg orally tiwce daily were commenced.
Fiftine years ago, Ms Bell had non-insulin-dependent diabetes mellitus. At present, encourage her to continue with medical hypoglycemic agent and antibiotic course prescribed. Monitor her ound margins assist in
saline dressing, observe signs of complication and prognosis need to encouraged.
Necessary education has been provided regarding medication compliance, regular physical exercise controlling blood sugsar level, restrict alcohol intake and also involvement of social activity adviced.
Based on the aforementioned circumstances, provide appropiate services it would be beneficial.
