Re: Mr Bill o' Riley, aged 50
Mr O' Riley, who recuperrating from coronary artery hipass grafts, is required low fat dietary guidelines and educate healthy simple recipes. This letter outlines his medical and social history.
On 2 September, Mr O' Riley was admitted to our hospital diagnosed as obstructive coronary artery disease. On 4 September surgery was performed. Postoperatiely, the condition was stable also good wound healing was
noticed. Therefore he is able to mobilize properly and perform regular exercise. Apart from, social worker daily visit him and discus the health condition.
Socially, Mr O' Riley is unmarried and lives alone in own house. He has habit of smoking 2o cigarettes per day as well as consumption alcohol 2 bottles at 300ml beer per day was repoted. His daily dietary regimen is
sausages, deep fried chips, eggs and McDonalds these are preferable diet. He has allergic towards nuts and BMI is 34.2.
On 15 September 2012 at 2pm, next follow up with general practitioner Dr Avril Tense has been scheduled along with local physiotherapist advised continue the rehabilitation exercise program.
Presently, low fat dietary guidelines and healthy simple recipes information will be sent to his home address. The address is 9476 Old DAM Road, Goondiwinidi, QLD 4390.
Based on the aforementioned circumstances, provide appropriate law fat dietary quidelines and healthy simple recipes it would be profoundly beneficial.
