Re: Mrs Maria John, aged 60

I am writing to refer Mrs John, who’s increassed TSH and elevated blood sugar requires expert assessment and treatment as she is being discharge today.


Mrs John admitted to hospital on 11 December with dizziness, abdominal pain associated with constipation. Apart from that she was experienced with slurred speech, increased thirsty and fatigue. On assessment vitals are elevated BP 170/110mm of Hg pulse rate 92/ mt and temperature and respiration within the normal.


After assessment a series of blood and radiological investigation done which shown raised level of T3, T4 blood sugar. Therefore, inj insulin, thuroxin 25 mg two times daily. In additionally coleum supplements tab pantoprazole 40mg daily at one time and paracetamol for pain prescribed. In radiology investigation revealed noted slight bleeding is temporal lobe and enlarged gland detected in neck.


Mrs John is married and lives with her husband. She has a history of stroke and has had hypertension as well as hyperthyroidism for the past 6 months. Also she ha been experiencing a seizure disorder as well.


Pertaining to the above mentioned information, kindly provide firther treatment based on assessment dor Mrs John. A physiotherapy followup for regular exercise and dietary modification including salt restricted calcium rich foods and limitation of diary products would be profoundly beneficial. She is rescheduled endocrinologist reisive after 2 weeks.
