Re: Mr George Gale, aged 85
I am writing to refer Mr Gale, into your facility for further care and management who is recovering from urinary tract infection. He is set to be discharge today.
Mr Gale admitted to the hosptial on 10 May with the history of fall due to weakness. On assessment his mental status was non stable, and vital signs were abnormal, urine analysis released and shown increased WBC which indicated above diagnosis consequently commenced with activities. During the days he done good progress with viral were stable.
Mr Gale is a widower lives alone in his flat. He drinks occasionally and smokes 10 cigarattes per day. He has been diagnosed with osteoarthritis, hypertension in 2003 and 2009 respectively for ehich he is on regular medication. In addition to that he has had non-specific colites which requires ongoing medications.
Given the above, it could be greatly appreciated if you could provide temporary residential care and support to Ms Gale. Kindly encouraged him to do 30 minutes of physical activity to improve her mobility. Please access his independent living before discharge from your facility because he is keen to go home and experiencing confusion.
