Dear Doctor,

Re: Albin Charlie, aged 4

I am writing regarding Albin who has been diagnosed with childhood autism, requires an urgent assessment and appropriate management at your service.

Albin is elder son of Ms Georgia. During my home visit it was observed that Albin has developed sorness and swelling at the injection site which resulted after yesterdays influenza vaccine. On examination he was febrile, hyperactive as well as presenting extent anger.

Albin started showing symptoms of development delay during his childhood. He started crawling at 10 month and poor speech and concentration was observed at the age of two. In addition, he also has no verbal or social skills and doesnot express feelings was found most of the time sitting alone crying and not showing interest in social activities as well as not maitaining physical relationship with his mother or siblings.

Albin lives with his mother and other siblings aged 2 and 6 months in a flat. His mother Ms Georgia lost her husband 10 months ago due to accident. She had consumed increased alcohol during pregnancy. Since it was intercast marriage. She does not have contact with her family members. She us anxious about his son’s anger speech delay and decreased social interaction.

As Albin’s mother is not able to take care of him, she has decided to move him to rehabilitation centre and would like to know more about the routines and admission process.

Based on the aforementioned circumstances, it would be beneficial if you could examine Albin’s current health status and provide management as per yout evaluation. In case any additional information is required, please do not hesitate to contact me.

Yours faithfully,

A Community Health Nurse.
