Re: Mr Walter Pitman, 69 years old
This letter is to update health status of Mr Pitman who requires a full cardiac assessment owing to his uncontrolled hypertension.
Mr Pitman was initially admitted to the newtown Hospital and was treated appropriately for accidental cut injury. On admission his wound was cleaned, dressed and sutured. He received the course of prophylactic antibiotics. Subsequent day his blood pressure was 140/90 mmHg and 180/90 mmHg in sitting and supine position respectively. He was discharged on third day and was referred to me for regular dressing and blood pressure monitoring.
In view of the above, it would be appreciates if you could perform a complete cardiac assessment of Mr Pitman. Kindly do the needful management for his hypertension.
