Dear Ms Petchell,
Ms Edien McGrath, who has had a foot ulcer for the past two weeks, requires assessment and appropriate management from your service.
Today, she presented with complaints of tiredness lethargy, tenderness, and swollen foot along with foot ulcer. On investigations, elevation of blood glucose level which was 240mg/dl, and was diagnosed with type 2 diabetes mellitus; therefore, Fortamet 1000mg has been commenced. X-ray was performed which showed osteomyelitis with grade 2 wound. As a result. Albactin 500 has been prescribed for the next ten days. Additionally, hypertension was managing with Atenolol 5mg once a day; however, medication compliance is irregular.
All the aspects of diabetic care was educated. A brochure about diabetic management was given and discussed. Refraining from weight-bearing, reducing alcohol consumption and adequate rest are recommended. A walking stick and well-cushioned footwear are suggested to use.
Based on the aforementioned circumstance, considering the wound, kindly evaluate her if she requires any surgical debridement procedure. A follow-up visit has been scheduled after one week following your consultation.
