Re: Mr Peter Hendrick, DOB:21 May 1972
Mr Peter Hendrick, who has been recuperating from angioplasty and stent insertion is being referred to your facility following his discharge. He requires home visit in order to provide advice regarding his daily living activities.
On 02 July 2019, Mr Hendrick was admitted to St Patrick's Hospital with definitive symptoms of myocardial infarction. Electrocardiogram revealed ST segment elevation along with 35% ejection fraction;therefore, he underwent angioplasty. During hospitalization, Physiotherapy as well as dietary consultations have been initiated. Health education regarding disease condition and lifestyle modifications have been rendered.
Mr Hendrick, a druck driver, married and has three children. His body weight is above the recommended level (BMI:29) and has dislipidaemia. He eats fast foods such as fries, hamburger, doughnuts and icecream. He also has family history of multiple heart diseases.
Pertaining to the above details, it would be appreciated if you could conduct home visit to provide recommendations for resuming his routine activities. Refraining from driving for the next six weeks is profoundly beneficial. It is essential to give suggestions regarding driving and returning to work. His detailed list of medications have been attached with this letter.
