Re: Ms Olivia Jack, DOB: 4 May 2000
Dear Director,
I am referring Ms Olivia Jack, who had delivered a baby through an emergency cesarean section, requires ongoing care and appropriate management from your service following the discharge today.
Postoperatively, the mother had no evidence of complications; however, initial period, the baby required resuscitation and regained normally after a few minutes. On observation, the mother is not confident about parenting skills and is also reluctant to provide breast milk. Additionally, she prefers bottle feeds. In addition, iron and vitamin C supplements have been commended as the mother had mild postpartum bleeding, currently, Hb is 90g/dl.
Ms Olivia jack lives alone, it was noted that, has no contact with family as well as the baby’s father. Therefore, isolation and insecured feeling were noticed, and also stated that the pregnancy was unwanted and wants to offer the baby for adoption.
Based on the aforementioned circumstances, Education regarding baby care and the importance of breastfeeding. Considering the mother’s situation regular home visits would be profoundly beneficial. Please note, at the time of the discharge baby’s weight is 3.7kg.
